Beginning August 1, 2026, Blue Zones Health will directly manage the core operational functions that support your practice. This is a deliberate move to align our infrastructure within a single, cohesive model grounded in Blue Zones principles and lifestyle medicine and integrated into everything we do.

If your office has not yet completed the Portal Registration and Cozeva Role Information forms, please do so as soon as possible. These forms are how we set up your portal access and Cozeva credentials. Without them your office will not receive login information the last week of July.

These changes affect how your office submits claims and gets paid. Please read this in full and share it with your billing and front-office staff.

What’s Changing on August 1

  1. Provider Portal. The new BZH provider portal is live at portal.bluezoneshealth.com. You will receive login credentials in a separate email. The ACTY portal remains open through September 14 for reference only. No new authorizations can be submitted through ACTY after July 31. All new authorization requests on or after August 1 must go through the new BZH portal.

    Portal training materials will be available in the portal starting July 29. If you need additional support, contact your rep. If you need to be set up in the portal, the registration form for initial setup is Form 1, and the form for adding additional staff is Form 3.
  2. Payments. If you were receiving EFT, you will now temporarily receive paper checks from now until end of year.
  3. Prior Authorization Requests. Use the Prior Authorization Request Form when submitting new authorization requests to Blue Zones Health.
  4. Customer Service. One number now covers both members and providers; callers are quickly routed to the right department: 833-LIV-BLUE. There will also be a Customer Service email function on the portal.
  5. Fax Number. Our new fax number is 323-574-0387.
  6. Claims. Submit claims through July 31 exactly as you do today. Beginning August 1, claims will be processed by Blue Zones Health. Blue Zones Health has partnered with Office Ally as our primary clearinghouse. See below for changes to Payer IDs effective August 1st. There is no transaction fee for Blue Zones Health providers to use Office Ally.

  7. Member Insurance Cards. Members may receive new insurance ID cards ahead of August 1. This is dependent on health plan issuance. Please update your EMR with the new information as soon as members present their new cards. This will prevent claim rejections at the payer level.
  8. Credentialing. Credentialing moves in-house on August 1. If your office has an open credentialing file that has not yet been processed, Blue Zones Health will take over that file directly. No action is needed on your end. Please send questions about credentialing to credentialing@bluezoneshealth.com.
  9. Medical Management. Blue Zones Health will take over utilization management and care management from ACTY. You will receive separate communication introducing the new UM and CM team and their contact process.
  10. Updating your location information, contact information, etc. Please email providerops@bluezoneshealth.com with any contact info change for our systems.

What Stays the Same

  • Patient care, coverage, and attribution. No changes, and no disruption to existing referrals.
  • Authorizations already approved remain valid, and nothing needs to be resubmitted. Auths submitted on 7/31 will be processed by ACTY, with decisions transferring to BZH within 2-3 business days. Until then, check status in the ACTY portal (available through 9/14) or call BZH Customer Service, who can route you to the UM team for up-to-date information.
  • Open issues carry over. Any issue your office has already reported will continue to be worked to resolution. You will not need to restart or re-explain it.
  • Your network participation continues. If any administrative contract updates are needed as part of this transition, we will contact you directly.
  • Cozeva workflows remain available. If your office uses Cozeva for care gaps, membership, or Annual Wellness Visits, the workflows remain available. You will be able to access Cozeva via the new portal.

Frequently Asked Questions

Using the Blue Zones Health Provider Portal

For all claims with dates of service July 31 and prior, should claims be submitted to the new BZH mailing address/Payer ID, or should they continue to be sent to ACTY?
The BZH mailing address/Payer ID. We’ve asked offices to submit all pending claims for dates of service July 31 and prior before July 31, but some may still come in after that date. If a claim for those dates isn’t submitted by July 31, starting August 1 it will need to go through the clearinghouse or be mailed to our mailing address. If it was submitted prior to August 1, you can find it in the ACTY portal through September 14, or call BZH. If it’s submitted after August 1, it will be in the Blue Zones Health Provider Portal.

Since EOBs are not generated in the provider portal, how will PCP offices obtain them?
By mail.

Will providers who are contracted but not yet credentialed be visible in EZ-NET?
Yes.

If the specialist or specialty a PCP is looking for is not available in EZ-NET, what is the next step?
Select “99 – Default” and use the notes field to provide more information on the requested provider.

Portal Registration

To request portal access, submit your information using the forms below. You will receive an automated notification acknowledging your submission, and your username and password within 48-72 hours once verified.

Well-Being Services

Well-Being Services is a clinically supervised extension of primary care that offers health coaching, nutrition counseling, social work support, care coordination, and lifestyle-based chronic disease management. Services are delivered both inside and outside the clinic through a team of credentialed professionals.

What Is a Well-Being Advisor?

A Well-Being Advisor plays a critical role in each member’s health journey. From virtual appointments to weekly group classes, members have multiple ways to interface with a Well-Being Advisor. Well-Being Advisors “prescribe” services and lifestyle habits, many of which are free to the member, and cover a wide range of needs such as managing A1C levels or addressing signs of loneliness. These referrals are made to specialists or classes that are virtually available or within the member’s Life Radius.
