EVENTS DISCLAIMER

Blue Zones Health (“BZH”) activities, classes, and events are offered for general educational, wellness, and community-support purposes.  They are not a substitute for professional medical advice, diagnosis, treatment, or emergency care.  Participation in an activity does not, by itself, create a patient-provider relationship, modify your health plan benefits or coverage, or replace any care plan or instructions provided by your physician, clinician, or BZH care team.

Before participating in any activity involving physical movement, exercise, nutrition changes, stress-management practices, or other lifestyle changes, consider your personal health conditions, medications, limitations, and abilities.  Consult your physician or another qualified health care professional as appropriate, especially if you are pregnant, have a chronic condition, have symptoms, take medications, have recently had surgery or an injury, or have been advised to limit physical activity.  Stop participating and seek medical attention if you experience chest pain, dizziness, fainting, severe shortness of breath, new or worsening symptoms, or any other concerning symptoms.  For a medical emergency, call 911.

Participation is voluntary.  Activities may be held at BZH clinics, community locations, third-party locations, or online, and may be led by BZH personnel, health professionals, community members, or independent third-party hosts or providers. Some activities, classes, events, instructors, hosts, venues, content, materials, products, or services may be provided, sponsored, operated, or made available by third parties that are independent of BZH. BZH does not control, supervise, endorse, or assume responsibility for third-party activities, providers, locations, content, practices, availability, safety protocols, privacy practices, or terms.  Your participation in third-party activities may be subject to additional terms, waivers, eligibility requirements, fees, cancellation policies, and privacy practices established by the third party.

Please review any applicable third-party terms and use your own judgment before participating.  Event details, availability, instructors, locations, and schedules may change.  BZH does not guarantee any specific health outcome, result, or improvement from participating in an activity.

Do not submit protected health information or other sensitive information through public or non-secure event forms, chats, or interactive features.  BZH’s use and disclosure of protected health information is governed by its Notice of Privacy Practices, where applicable, and online privacy practices are described in its Internet Privacy Policy.

BZH is committed to providing accessible activities and effective communication.  If you need an accommodation, auxiliary aid or service, language assistance, or information in an alternative format, please contact us before the activity so we can make reasonable arrangements.
