Provider Onboarding & Orientation

Welcome to Blue Zones Health

We’re glad you’re here. This page walks you through what to expect as a new BZH provider, from signing your contract to your first weeks of seeing patients.

Your Onboarding Journey

Part 1: Onboarding

  1. Signing on. Your practice signs its agreement with Blue Zones Health.
  2. Sharing your roster. We’ll ask for a roster of providers at your practice. Send it back and credentialing begins on our end.
  3. Requesting portal access. Your practice submits the Portal & Cozeva Registration Form to request portal access for any staff who need it, and to register for Cozeva.

Part 2: Orientation

  1. The credentialing wait. Credentialing review wraps up during this stretch, and health plan approval begins. This takes an average of two weeks on our end, though it can vary depending on how quickly we hear back from you with any requested information.
  2. Getting ready. Before go-live, your team watches the BZH welcome video and orientation series below. The pledge form is coming soon.

What Happens Next

Once orientation wraps up, a few more things happen on the way to go-live and beyond: you’ll receive your health plan provider IDs and Provider Portal login credentials, and we’ll schedule a pre-onboarding meeting followed by your full onboarding meeting, which covers portal training, your Provider Quick Reference guide, the AWV program, authorizations, who to contact for UM, and Well-Being Services. About a week after your go-live date, we’ll check in to talk through patient enrollment, any authorization issues, and available classes.
